How to Save/Print Enrollment List & E-Forms

How to Print Enrollee List

Here’s a step-by-step guide to view and print a list of enrollees for a specific month:

  1. Go to See Program Results.
  2. Select the Enrollment List Icon.
  3. Select the Month and Year from the drop-down.
  4. Select View All at the bottom to view all participants on one page.

5. To print the form:  click the Print button and follow your printer’s prompts.

6. To save as a PDF, click the Print button, then change the printer destination to “Save as PDF.” Click Save, name your file, and select where to save it on your computer.

    How to Print/Save E-Forms (Income Eligibility & Enrollment Forms)

    Please note: you will need to do these for each individual participant.

    1. Go to Manage Enrollment.
    2. Select View Current List.
    3. Click on the Participant’s Name to open their details.

    4. In the Eligibility Details tab, find the Attached Files section (bottom right).

    5. Click on the Digital Signature link to open the E-Form.

      6. To save the form: Right-click on the E-Form and select Save As. Name the file and save it to your computer.

      7. Or, click the print icon on the top of the file to print and follow your printer prompts.
